Room of Errors: Recognizing Risk Before Harm Happens

Think you can spot the safety concerns hidden in a patient care environment?
UAB Clinical Simulation’s Virtual Room of Errors gives learners the opportunity to test their safety awareness in an interactive 360° experience.
As part of Quality Week 2026, the Virtual Room of Errors will be open Oct. 19 at 8 a.m. through Oct. 26 at 8 a.m. During this time, UAB Hospital units/areas are invited to participate in a friendly competition to see which has the highest participation.
This interactive experience challenges learners, whether clinical or non-clinical, to identify safety concerns within their role in a patient care environment. Using a 360° virtual room, learners can click and drag to inspect the space, recognize potential hazards, consider the significance of each finding and choose the safest next action.
Room of Errors focuses on visible risks that may affect patient and staff safety, including environmental hazards, infection prevention concerns, equipment readiness, fall prevention, invasive devices, restraints and patient identification. Rather than simply asking learners to “spot the error,” the experience encourages safety-focused thinking: recognize the concern, interpret the risk, prioritize what matters most, act within role and scope, reassess and escalate when needed.
This experience can be used as a refresher, onboarding activity, staff meeting discussion or targeted learning opportunity for teams wanting to strengthen hazard recognition and patient safety awareness.
Room of Errors can be accessed via the Clinical Simulation website or through UAB Medicine Quality Week information and flyers posted around campus.
